You will find the link to the survey at the bottom of the page.
You will soon be able to choose between the recorder or the check as promised

The confirmation page for the survey is the page with all the instructions. Enter your shipping info (even if there are identical to the info entered at the beginning of the survey), re-enter your woot confirmation number, and make sure to choose between the recorder and the rebate. Also make sure to print this page.
Note that in question Number 40 (shipping information) we ask you for your Firstname, Lastname, and there is another field where we ask for your Name once more (just to make sure), please enter your full name in this field as well. You will not be able to finish the survey if you don't. As I mentionned above, make sure to print this page.

| Sorry, but who keeps the confirmation number? I dont have it any longer. Once I received the package that confirmation email was history. |

Log into Woot.com, click on "your account" and you will see your list of items purchased, which includes your confirmation number for your Neuros 442 order.

Luckily Woot saves all the receipts for you under your account history.
Its a good practice to save your receipts/confirmations from online purchases as you will need these as proof of purchase for most warranty repair or replacement.

Thanks for the location of my number. That makes my Recorder closer to being home.
I thought as long as I paid using a Credit Card that was my proof of purchase. |

In general a Credit card receipt is only going to show a total dollar amount of your purchase from a retailer. Its not going to show The actual products you purchased. |

| I seem to be having trouble filling out the survey. No matter how low I set my security settings, the website still seems to be unable to save coookies to my computer. |

Is that after you send in your confirmation page? Or after the posting of the survey? For some of us lazy folks who originally couldn't wait to get our rebate (the recorder, in my case), there might be a difference!

If you sent the page with Questions 38, 39 and 40 along with the original UPC code (not a photocopy), you are all set.
